Initial Comments:
An off-site follow-up survey completed on December 22, 2015, found that Hamilton Health Center Inc., had corrected the deficiencies cited under 42 CFR, Part 405, Subpart X and 42 CFR Part 491.1- 491.11 Subpart A, Conditions for Certification: Rural Health Clinics and Federally Qualified Health Center (FQHC) Services. The deficiencies were cited as a result of a Medicare complaint investigation survey completed on 10/8/15.
